Состояние тканей периодонта у пациентов с воспалительной патологией кишечника

Целый ряд доказательств взаимоотягощающего влияния патологии периодонта и кишечника через многочисленные микробиологические, иммунологические, психологические и метаболические ассоциации определяет повышенный интерес к проблеме патологии периодонта у пациентов с воспалительными заболеваниями кишечника (ВЗК).
Этиопатогенез ВЗК, т. е. язвенного колита и болезни Крона, во многом сходен с периодонтитом. Оба заболевания связаны с чрезмерной воспалительной реакцией слизистой оболочки кишечника или полости рта на микробный триггер у восприимчивого хозяина.
Цель исследования – оценить тяжесть клинического течения патологии периодонта у пациентов с ВЗК.
ВЗК и периодонтит являются патологией воспалительного генеза, хронического, прогрессирующего течения, они объединены общностью патогенеза, заключающегося во взаимодействии и взаимоусугублении нарушений микробиоценоза и иммуновоспалительных реакций. ВЗК – один из важнейших факторов риска развития и тяжелого течения периодонтита. Нами установлено, что у пациентов с ВЗК риск тяжелого пародонтита ΙΙΙ–ΙV степени составляет 2,431 (95 % ДИ 1,704–3,470), а этиологическая доля вклада ВЗК в риск периодонтита ΙΙΙ–ΙV степени равна 58,86 %. Наличие фактора риска увеличивает тяжесть патологии периодонта.
Достоверно более высокая распространенность периодонтита была обнаружена у пациентов с ВЗК по сравнению с группой сравнения – 94,16 % против 83,97 % ( p < 0,001).
