Инфекционный статус детей после трансплантации почки и опыт применения этиотропного лечения вирусных осложнений

Аннотация

В статье изучены инфекционный статус детей в разные сроки после трансплантации почки и опыт этиотропного лечения посттрансплантационных осложнений. Приведены данные генодиагностики вирусных инфекций, возбудителями которых являются цитомегаловирус, полиомавирусы (ВКV, JCV), вирус Эпштейна–Барр, варицелла-зостер вирус, вирусы простого герпеса 1-го и 2-го типов, вирусы герпеса человека 6-го и 7-го типов, аденовирусы. Установлено, что частота их регистрации при исследовании крови в раннем послеоперационном периоде (первые 3 мес. после пересадки) составила 20,0 %, в позднем – 8,0, в условиях дисфункции трансплантата – 21,4 %. По результатам ПЦР-исследований мочи ВКV и/или JCV вирурия регистрировалась у 40,0 % реципиентов в раннем послеоперационном периоде, у 47,1 % – в позднем. Доминирующими возбудителями были полиомавирусы. При этом в раннем периоде преобладали ВКV (22,9 %), в позднем – JCV (36,8 %). Приведены примеры лечения острого отторжения трансплантата на фоне виремии ВГЧ 6 и дисфункции графта на фоне длительного персистирования ВКV инфекции. Применение валганцикловира в первом случае и перевод пациента с мофетила микофенолата на азатиоприн во втором случае позволили стабилизировать функцию пересаженного органа.
