При тяжелом течении заболевание характеризуется развитием цитокинового шторма, который приводит к развитию синдрома полиорганной дисфункции и острого респираторного дистресс-синдрома (ОРДС), обусловливающих высокую летальность. Для подавления цитокиновой агрессии у пациентов с тяжелым течением COVID-19 рекомендована медикаментозная супрессия, однако в качестве альтернативного метода патогенетически может быть применена и гемосорбция.</p><p>Целью исследования являлась оценка эффективности гемосорбции с использованием гемосорбента «Гемо-Протеазосорб» по сравнению с медикаментозной супрессией тоцилизумабом при тяжелом течении COVID-19.</p><p>Изучены клинико-лабораторные показатели 88 пациентов, находившихся на лечении в отделении анестезиологии-реанимации. Все пациенты были разделены на две группы: Гемо-Протеазосорб (n = 53) и Тоцилизумаб (n = 33).</p><p>На фоне проведения гемосорбции с использованием сорбента «Гемо-Протеазосорб» отмечено статистически значимое снижение уровня прокальцитонина, С-реактивного белка, лейкоцитов, фибриногена, Д-димеров, повышение числа лимфоцитов, а также более раннее повышение респираторного индекса, чем в группе, где проводилась терапия тоцилизумабом. В ходе исследования установлено, что использование отечественного сорбента «Гемо-Протеазосорб» имеет ряд преимуществ перед медикаментозной супрессией цитокинового шторма тоцилизумабом ввиду более выраженных эффектов иммуномодуляции, улучшения кислородтранспортной функции крови, ее реологических свойств и возможности эффективного применения у лиц с вирусно-бактериальной инфекцией.</p></abstract><trans-abstract xml:lang="en"><p>Coronavirus infection is a clinical syndrome caused by a mutational RNA virus (SARS-CoV-2). In severe cases, it is characterized by the development of a “cytokine storm”. The latter leads to the multiple organ dysfunction, ARDS and causes high mortality. To suppress the cytokine aggression in patients with severe COVID-19, the drug suppression is recommended; however, the use of hemosorption can be pathogenetically justified as an alternative method.</p><p>The aim of the study was to assess the effectiveness of hemosorption using the hemosorbent “Hemo-Proteazosorb” in the severe COVID-19 infection course in comparison to the drug suppression with tocilizumab.</p><p>We studied the clinical and laboratory parameters of 88 patients who were treated at the anesthesiology and intensive care department. All patients were divided into 2 groups: Hemo-Proteazosorb (n = 53) and Tocilizumab (n = 33).4</p><p>Against the background of hemosorption using the Hemo-Proteazosorb sorbent, statistically the level of procalcitonin, C-reactive protein, leukocytes, fibrinogen, D-dimers significantly decreases, the number of lymphocytes increases, as well as the respiratory index earlier grows in comparison with the group receiving tocilizumab therapy.
