Необходимость своевременной профилактики и коррекции плацентарных нарушений у беременных при варикозном расширении вен нижних конечностей диктует необходимость разработки способа прогнозирования указанного осложнения гестации, основанного на доступных и информативных современных показателях, включая биохимические и гемостазиологические маркеры эндотелиальной дисфункции.Цель исследования – улучшить гестационные исходы у беременных при хронических заболеваниях вен нижних конечностей на основе разработанного метода прогнозирования и ранней диагностики плацентарных нарушений.Для разработки математической модели прогнозирования плацентарных нарушений при варикозном расширении вен нижних конечностей обследовано 128 беременных с хроническими заболеваниями вен нижних конечностей, которым в сроке 22–24 недели проведена комплексная оценка клинико-лабораторных показателей, включая определение уровней сосудисто-эндотелиального фактора роста (СЭФР), эндотелина-1, С-реактивного белка (СРБ), витамина 25(ОН)D, оценку системы гемостаза и ультразвуковое исследование маточно-плацентарного кровотока.Установлено, что в структуре гестационных осложнений при варикозном расширении вен нижних конечностей ведущее место отводится плацентарным нарушениям – 36,7 %. На основе обобщенных линейных моделей с логистической функцией связи и анализа ROC-кривых определены наиболее информативные предикторы развития хронической плацентарной недостаточности (уровни СЭФР, эндотелина-1, СРБ, фибриногена, Д-димеров), включая класс тяжести хронических заболеваний вен нижних конечностей по СЕАР (AUC = 0,94; 95 % ДИ 0,842‒0,956; p &lt; 0,001), и с учетом комплекса клинико-лабораторных параметров разработана математическая модель, позволяющая прогнозировать развитие плацентарных нарушений у беременных с варикозным расширением вен нижних конечностей.</p></abstract><trans-abstract xml:lang="en"><p>The objective of the study was to identify predictive biomarkers and generate the model to predict placental lesions in women with varicose veins. We collected serial serum specimens from 128 women with varicose veins between 22 and 24 weeks’ gestation. The investigation includes ultrasound findings, blood analysis of endothelin-1, vascular endothelial growth factor (VEGF), CRP, coagulation factors as well as BMI. We used machine learning algorithm and multivariable logistic regression with Lasso method to predict placental lesions among the pregnant patients with varicose veins.A total of 47 (36.7 %) women with varicose veins subsequently developed placental insufficiency. Mean serum VEGF were higher in women who developed placental insufficiency – 29 (27–31) pg/ml, as compared with women without varicose disease – 24 (22–25) pg/ml, p &lt; 0.001.
