Показано, что механизмы клеточного повреждения многокомпонентны и тесно взаимосвязаны регуляторными факторами молекулярного уровня. На ранней стадии заболевания они приводят к следующим структурно-функциональным изменениям ацинарных клеток, способствующим преждевременной внутриклеточной активации трипсиногена и аутоагрессии: устойчивому подъему цитозольного Ca2+ и избытку митохондриального Cа2+, дестабилизации лизосом и зимогенных гранул, нарушению аутофагии, деполяризации митохондрий, снижению выработки АТФ и некрозу.</p></abstract><trans-abstract xml:lang="en"><p>The review analyzes the current data on the main mechanisms of toxic effects of alcohol and its metabolites on pancreatic acinar cells in acute pancreatitis. It is shown that the mechanisms of cellular damage are multicomponent and closely linked by the regulatory factors of the molecular level. At the early stage of the disease, they lead to the following structural and functional changes in acinar cells that promote the premature intracellular trypsinogen activation and autoaggression: sustained rise of cytosolic Ca2+ and excess of mitochondrial matrix Ca2+; destabilization due to lysosomes and zymogen granules; debective autophagy; mitochondrial depolarization; decreased ATP production and necrosis.</p></trans-abstract><kwd-group xml:lang="ru"><kwd>поджелудочная железа</kwd><kwd>ацинарные клетки</kwd><kwd>острый панкреатит</kwd><kwd>алкоголь</kwd></kwd-group><kwd-group xml:lang="en"><kwd>pancreas</kwd><kwd>acinar cells</kwd><kwd>acute pancreatitis</kwd><kwd>alcohol</kwd></kwd-group></article-meta></front><back><ref-list><title>References</title><ref id="cit1"><label>1</label><citation-alternatives><mixed-citation xml:lang="ru">Острый панкреатит.
