В то же время данные о фармакокинетике колистина, необходимые для оптимизации его дозирования, в литературе представлены недостаточно. Изучена фармакокинетика колистина и колистиметата натрия (КСМ) у детей с химиоиндуцированной нейтропенией. Для количественного определения колистина в сыворотке крови был применен метод высокоэффективной жидкостной хроматографии (ВЭжХ) с масс-спектрометрией. Определена концентрация колистина после внутривенного введения КСМ у 21 ребенка (13 пациентов с сепсисом, 8 детей контрольной группы) с химиоиндуцированной нейтропенией. Выявлена существенная вариабельность фармакокинетических параметров колистина как у пациентов с сепсисом, так и у лиц контрольной группы. Метод ВЭжХ с масс-спектрометрией можно использовать для терапевтического лекарственного мониторинга и оптимизации режима дозирования.</p></abstract><trans-abstract xml:lang="en"><p>The spread of nosocomial multidrug-resistant strains of P. aeruginosa, A. baumannii and K. pneumoniae brought back the need of colistin use. However, the data concerning pharmacokinetics of colistin is lacking, precluding from optimal dosing of the drug, especially in critically ill patients.The pharmacokinetics of colistin and colistimethate sodium in children with chemotherapy-induced neutropenia was studied. The serum concentration of colistin was determined by high performance liquid chromatography (HPLC) and mass spectrometry. The concentration of colistin after intravenous administration of colistimethate sodium was determined in 21 children (13 patients with sepsis, 8 children in the control group) with chemotherapy-induced neutropenia.The significant variability of pharmacokinetic parameters of colistin in patients with sepsis and in the control group was revealed.
