Изучены частота и факторы риска неблагоприятных исходов у 400 пациентов с постинфарктным кардиосклерозом. Установлено, что в течение года повторный инфаркт миокарда (ИМ) развился у 92 (23 %) пациентов, умерло 26 (6,5 %). В группе пациентов с повторным ИМ отмечалось большее число лиц с депрессивными расстройствами, что ассоциировано с низким уровнем личностной тревожности, передней локализацией ИМ, достоверным снижением контрактильной функции левого желудочка и увеличением размеров миокарда левого желудочка, более частым развитием тахиаритмий, а также с увеличением концентрации провоспалительных цитокинов. Для пациентов со смертельным исходом были характерны семейный анамнез ранней ишемической болезни сердца, преобладание лиц с ситуационной тревожностью, более частое развитие тахиаритмий, снижение сократительной способности и ре-моделирование миокарда левого желудочка, более высокие значения общего холестерола и липопротеинов низкой плотности, а также концентрации NT-фрагмента мозгового натрийуретического пептида.

This article is devoted to the study of the frequency and risk factors for adverse outcomes among patients with postinfarction cardiosclerosis. The study included 400 patients with a history of first myocardial infarction. The study found that within a year, recurrent myocardial infarction occurred in 92 (23 %) patients died of 26 (6.5 %) patients. The group of patients with recurrent myocardial infarction has a large number of people with depressive disorders that are associated with low levels of trait anxiety, anterior localization of myocardial infarction, a significant decrease in the contractile function of the left ventricle and the enlargement of the left ventricular myocardium, more frequent development of tachyarrhythmias, as well as an increase in the concentration of pro-inflammatory cytokines. For the group of patients with lethal characteristic family history of early coronary artery disease, the prevalence of individuals with situational anxiety, more frequent development of tachyarrhythmias, reduced the contractility of the myocardium and the left ventricular remodeling, higher values of total cholesterol and low-density lipoprotein, as well as the concentration of NT-fragment brain natriuretic peptide.
