Для оценки уровня системного воспаления у доноров со смертью мозга можно использовать различные лабораторные маркеры.</p><p>Целью данного исследования стало изучение динамики системного воспаления по уровню С-реактивного белка (СРБ) и сывороточного железа у потенциальных доноров органов и тканей во время проведения протективной интенсивной терапии.</p><p>В проспективное нерандомизированное исследование было включено 404 потенциальных донора органов и тканей со смертью мозга, которым проводилась протективная интенсивная терапия и кондиционирование функциональных систем. Основными диагнозами, которые приводили к смерти головного мозга, были следующие: нетравматические внутричерепные кровоизлияния (n = 259 (64 %)); ишемическое повреждение головного мозга – инфаркт мозга (n = 63 (16 %)); постгипоксическая энцефалопатия (n = 16 (4 %)); черепно-мозговая травма (n = 66 (16 %)).</p><p>У потенциальных доноров с черепно-мозговыми травмами наблюдался более высокий уровень СРБ и более низкий уровень железа, что может быть обусловлено такими причинами, как проводимые оперативные вмешательства, кровопотеря, более отсроченное наступление смерти мозга и более длительный срок интенсивной терапии. Была выявлена положительная корреляция между уровнем системного воспаления, оцениваемого по СРБ и сывороточному железу, и наличием у донора ожирения (ИМТ – 30 кг/м2 и более): R = 0,24, р = 0,0009 и R = 0,17, р = 0,02 соответственно. Корреляционный анализ выявил наличие статистически значимой корреляции между уровнем СРБ и сывороточного железа и длительностью интенсивной терапии до начала констатации смерти мозга: R = 0,42, p &lt; 0,00001 и R = –0,23, р = 0,0002 соответственно.</p><p>Уровень сывороточного железа и уровень СРБ – это независимые маркеры системного воспалительного ответа у потенциальных доноров с необратимым повреждением мозга.</p></abstract><trans-abstract xml:lang="en"><p>The dynamics of the systemic inflammatory response in potential donors with irreversible brain injury and death have not yet been reliably established. Various laboratory markers can be used to assess the level of systemic inflammation in brain-dead donors.</p><p>The aim of this study was to evaluate the dynamics of systemic inflammation based on C-reactive protein (CRP) and serum iron levels in potential organ and tissue donors during protective intensive care.</p><p>This prospective, non-randomized study included 404 potential brain-dead organ and tissue donors, who underwent protective intensive care and functional systems conditioning. The main diagnoses leading to brain death were as follows: non-traumatic intracranial hemorrhage (n = 259 (64 %)); ischemic brain injury – cerebral infarction (n = 63 (16 %)); posthypoxic encephalopathy (n = 16 (4 %)); traumatic brain injury (n = 66 (16 %)).</p><p>Potential donors with traumatic brain injury had higher CRP levels and lower iron levels, which may be due to factors such as surgical interventions, blood loss, delayed brain death, and a longer duration of intensive care. A positive correlation was found between the level of systemic inflammation, assessed by CRP and serum iron, as well as the presence of obesity (BMI – 30 kg/m2 or greater) in the donor: R = 0.24, p = 0.0009 and R = 0.17, p = 0.02, respectively.
